Overview

Helicobacter pylori Selective Medium Base is a specialised culture medium for the isolation and cultivation of Helicobacter pylori, a microaerophilic Gram-negative spiral bacterium and the primary causative agent of chronic active gastritis, peptic ulcer disease, and gastric adenocarcinoma. Classified as an IARC Group I carcinogen, H. pylori colonises the human gastric mucosa and requires enriched, highly selective conditions for successful primary isolation.

The base medium is Columbia Blood Agar supplemented with 5–10% defibrinated sheep blood (providing haemin, NAD, and growth factors) and a four-antibiotic selective cocktail: vancomycin (Gram-positive suppression), trimethoprim (enteric flora), cefsulodin (anti-Pseudomonas), and amphotericin B (antifungal). H. pylori is intrinsically resistant to all four antibiotics at concentrations used, permitting preferential recovery from complex clinical specimens.

Preparation Protocol
  1. 1Suspend 39 g of dehydrated Columbia base in 1 L of purified water.
  2. 2Heat with agitation until completely dissolved. Bring to a rolling boil for 1 minute.
  3. 3Autoclave at 121 °C (15 psi) for 15 minutes.
  4. 4Cool to exactly 45–50 °C. Critical: above 50 °C will lyse blood cells.
  5. 5Add 5–10% v/v defibrinated sheep blood (pre-warmed to 45 °C) aseptically. Mix gently — avoid frothing.
  6. 6Add antibiotic selective supplement aseptically per manufacturer's instructions. Mix gently.
  7. 7Pour 15–20 mL per 90 mm plate under laminar flow. Allow to solidify.
  8. 8Dry at 37 °C for 30–45 min. Store at 2–8 °C in sealed bags. Use within 2 weeks.
