Mechanism of Action: Ceftriaxone irreversibly binds to and inhibits transpeptidase enzymes — specifically penicillin-binding proteins (PBPs 1a, 1b, 2, and 3) — that catalyse the final cross-linking step in peptidoglycan biosynthesis. Blockade of PBPs prevents cell wall maturation, leading to structural instability, osmotic imbalance, and ultimately bactericidal cell lysis. Its extended half-life is attributed to high plasma protein binding and resistance to many β-lactamases.
